INITIAL MANAGEMENT SUMMARY

MAJOR BURNS: INITIAL MANAGEMENT TREATMENT SUMMARY

  1. STOP THE BURNING PROCESS
  • Water for smoldering clothing
  • Water for chemical burns
  • Remove clothing - keep warm
  • Cool water for small 2° burns only
  1. ASSURE ADEQUACY OF VENTILATION AND OXYGENATION
  • Provide oxygen for all burns to treat carbon monoxide
  • Consider early endotracheal intubation with smoke inhalation injury
  1. INITIATE RESTORATION OF HEMODYNAMIC STABILITY SYSTEMICALLY & LOCALLY
  • isotonic crystalloid infusion
  • Remove any constricting items
  • Consider Escharotomy for circumferential burns
  1. LOOK FOR OTHER TRAUMATIC INJURIES
     5.      BURN WOUND LAST PRIORITY
 


Blunt traumatic injury in major burn patient caused by a car fire after collision. Priority of C-spine injury exceeds that of burn. Burned clothing can retain heat and needs to be removed.
