Section 3c.  Collagen Dressings

 

  1. PROMOGRAN® (Matrix Wound Dressing) [Johnson & Johnson]

Description:

Promogram collagen dressing is made by combining animal collagen (55%) with oxidized regenerated cellulose (45%).  The collagen structure is manufactured.  The cellulose provides absorptive properties.  The ORC/collagen is reported to absorb excess proteinases from the wound surface.  There are no glycosaminogklycans added or other dermal elements. 

The ORC/collagen dressing is reported to create a collagen environment attracting cells, and supporting tissue growth.

 

Indications:  Indicated for most partial and full thickness wounds

Healing Environment:

A moist wound healing environment is maintained.

How Applied:

Debride any surface necrotic tissue and treat any infection.  Hydrate the Promogran with saline for wounds with minimal exudates.  The collagen cellulose layer becomes a surface gel.  Reapplication can be performed every 2-3 days.  Reapply several times as the Promogran either disappears or is incorporated.  There is no data indicating incorporation in the wound.  The cellulose  layer is likely degraded.  Apply secondary gauze dressing over Promogran.

  1. PURAPLY® (Collagen Dressing) [Royce Medical]

Description:

PuraPly is a dressing composed of a reconstructed type I collagen (Porcine) biomaterial.  A 3 dimensional collagen reconstructed structure, free of cells is present.  There are no other biologically matrix elements present.  The product is exposed to acids and bases as well as gamma sterilization during production.

 

Indications:  The dressing is indicated for the management of most partial and full thickness wounds including chronic wounds, donor sites and for MOHS surgery. 

Healing Environment:  Moist wound healing is maintained.

How Applied: The Puraply is supplied as a single layer (75 microns in thickness) dry fenestrated sheet.  The wound is debrided and the PuraPly is cut to size and applied then moistened if the wound itself is not already moist.  A secondary dressing is applied to hold in place.  PuraPly can be applied as layers although the timing of re-application is not well described.

 

  1. FIBROCOL PLUS® (Collagen Dressing) [Johnson & Johnson]

Description:

Fibrocal is a collagen dressing made by combining animal collagen 90% with an alginate 10%.  The collagen sheet is made of a denatured collagen lattice.  The lattice provides the wound healing property while the alginate provides the exudate absorption. 

The collagen lattice plus the surface moisture is considered to create a stable environment for wound healing.  There are no other biologically active proteins or matrix components present.  The product claims to improve autolytic debridement on wounds with surface dead tissue.  Also Fibrocal is reported to decrease pain compared to a hydrocolloid. The product is not incorporated into the wound.

 

Indications:  Current indications are similar to OASIS, being partial thickness wounds, venous ulcers, acute traumatic wounds, second degree burns

Healing Environment:   Moist wound healing.

Application: Cut the Fibracol to fit, then apply to moistened wound bed, followed by an absorbent secondary dressing.  A gel forms which remains on the wound during the healing process.
